B-52 pilot Arthur Holland aka Bud steers a B-52 Stratofortress beyond operational parameters resulting in a crash, killing all 4 crew members at the age of 46 in Fairchild Air Force Base, Fairchild, Washington, United States.

In an investigation the USAF stated that the crash was entirely attributable to Holland's personality and behaviour.

281 die in passenger and goods train collision in Igandu, Dodoma, Dodoma, Tanzania.

The driver stopped the train near the summit of the hill to inspect and adjust the braking system and climbed back into the cab. When he started the train the brakes totally failed, causing the train to roll with great velocity straight back down the hill, crashing after passing two stations into a slow moving goods train.

I took off my left shoe and tied small branches and straw against my leg. I lit several candles together and let the candlewax drip over the branches and my leg until a kind of shell was formed. Then I cut loose all the ropes and left the small stake behind.
